My project focuses on conducting a Program Evaluation Plan for the Assertive Community Treatment (ACT) Model. The ACT model is a community-based interdisciplinary recovery model for individuals with severe mental health disorders. Conducting this assessment will further identify determinants of health, determining Evidence-Based Practice interventions to close these gaps in care. Further focusing on collaborative efforts, and the support of internal and external stakeholders will allow for increased advocacy for this vulnerable population and further increase access to care. This addresses the course objective of Formulate public health assessments and interventions for improving quality health outcomes for populations in their selected setting. The utilization of a concept map can better organize the key points of the information gathered. This also provides another option to communicate the information, making it easier to understand for those that prefer visual learning. A concept map can also assist in connecting ideas and conceptualizing how the ACT Model can best be utilized to provide holistic, evidence-based care. This can also be useful in identifying further gaps in care and resources available.
